Rate each item on a scale from 1 to 5. Go through the items together with employees or their representatives. Answer "don't know" if it needs to be looked into.

Step 1: Identify the hazards. Include every condition that exists in your workplace, both physical and psychosocial. You choose the method.

Describe and assess

Here are all items rated 2 or more, or that need looking into. Describe the problem, and assess the probability of injury or ill health and how serious the consequences could be.

Step 2: Describe and assess the problems found. The scale from 1 to 5 shows how big the problem is experienced to be. The risk matrix shows how serious the risk is, so a rare problem with serious consequences can still get high priority.
